3-3.1 Physical Exam and Vision Evaluation Physical Exam: 1. Admission to school requires submission of evidence of a physical examination within six months prior to entrance into the beginner grade (Kindergarten or, if Kindergarten is not attended, the first grade) and in the case of a transfer from out of state, to any grade. 2. Evidence of a physical examination is also required within six months prior to entrance in the seventh grade.
3. The physical examination is to be performed by a physician, a physician assistant or a nurse practitioner.
Vision Evaluation: 1. Admission to school requires submission of evidence of a visual evaluation within six months prior to entrance into the beginner grade (Kindergarten or, if Kindergarten is not attended, the first grade) and in the case of a transfer from out of state, to any grade.
2. The visual evaluation is to consist of testing for amblyopia, strabismus and internal and external eye health, with testing sufficient to determine visual acuity.
3. The visual evaluation is to be performed by a physician, a physician assistant, an advanced practice registered nurse or an optometrist.
The cost of the physical examination and vision evaluation shall be borne by the parent or guardian. The parent or guardian shall be notified in writing of the foregoing requirements and of the right to submit statements to object to the requirements. The parent or guardian shall also be provided a telephone number or other contact information to assist the parent or guardian in receiving information regarding free or reduced-cost visual evaluations for low-income families who qualify.
A student who fails to meet the foregoing requirements shall not be permitted to enroll or to enter school, or if provisionally enrolled or enrolled without compliance, shall not be permitted to continue in school until evidence of compliance or an exemption from compliance is given.

3-3.2 Onset of Illness or Accident in School
If any pupil develops
symptoms of illness or is injured at school, the parent, guardian, or
other person designated on the pupil’s enrollment information as an
emergency contact by the parent shall be notified immediately and, if
deemed advisable by the school, such person shall be requested to come
to the school and get the child. If no person on the enrollment
information emergency contact list is reachable by phone or in person,
the student must remain in school until someone is reached. In
case of dire emergency the school may call the physician designated in
the pupil’s enrollment information, or contact the school nurse for
administration of temporary relief or aid unless the parent or guardian
expressly forbids.

3-3.3 Dispensation of Medicine
The Board of Education
believes that the responsibility for prescription, possession and
administration of a student’s medicine lies with the parent and
physician. The schools will not regularly provide medication for
students, except in emergency situations which may occur during the
regular school day or physician directed situations accompanied by a
release of liability from the students parents or legal guardians.

3-3.4 Proof of Immunization Prior to enrollment, each Nebraska City Public Schools student shall provide a written immunization history, signed by the student’s physician, parent or guardian, verifying that the student has received the required vaccines so as to be protected by immunization.
Any student who does not comply shall not be permitted to enter school, or if provisionally enrolled or enrolled without compliance, shall not be permitted to continue in school until evidence of compliance or provisional requirements continue to be pursued or an exemption from compliance is given.
Immunizations will not be required for a student’s enrollment if the student submits one of the following:
(A) A statement signed by a physician, physician assistant, or nurse practitioner that, in the health care provider’s opinion, the immunizations required would be injurious to the health and well being of the student or any member of the student’s family or household;
(B) An affidavit signed by the student, or if the student is a minor, by a legally authorized representative of the student, stating that the immunization conflicts with the tenets and practice of a recognized religious denomination of which the student is an adherent or member or that immunization conflicts with the personal and sincerely followed religious beliefs of the student.
(C) Documentation from the student’s immunization provider showing that the student has begun the required immunizations and has scheduled dates to complete the immunization series as rapidly as is medically feasible. This documentation will permit provisional enrollment only.
The cost of immunizations and any required documentation shall be borne by the parent or guardian, not the Nebraska City Public School district.

3-3.5 Pregnancy
The school my not apply any
rule concerning a student’s actual or potential parental, family, or
marital stature which treats students differently on the basis of sex.
No student may be discriminated
against or excluded from an educational program or activity (including
any class or extra-curricular activity) on the basis of pregnancy or
related conditions. A student may voluntarily request to
participate in a separate portion of the program or activity.
The decision for participation will
take into account the health and safety issues of the activity in
relationship to the pregnancy. The school system has the right to
request a doctor’s approval regarding participation in any particular
activity by a pregnant student.
Student pregnancy and related conditions will be treated as any other temporary disability.

3-3.6 Communicable Disease Control
The Nebraska City Public
Schools shall cooperate with the county and state health departments in
developing procedures for the control of communicable disease in the
schools. All procedures shall conform to the regulations for
communicable disease control set up by the State Health
Department. In general, a student with a communicable condition
will be allowed to attend school in his/her usual class setting with
the written approval of the student’s physician stating that the
disease is not in a communicable stage. Without such a written
statement, a student with a communicable condition is subject to an
emergency exclusion.
When a child is sent home because of
a suspected reportable communicable disease, a report will be provided
to the State Department of Health without delay.

3-3.7 Automated External Defibrillator (AED) Policy
In order to enhance safety measures for the staff members, students and visitors of the Nebraska City Public Schools, it
shall be the policy of the Nebraska City Public Schools to maintain an Automated External
Defibrillator (AED) at all of the Nebraska City Public Schools.
Administrative Regulation sets forth the practice protocols and procedures of the AED program.
Use of the AED in an emergency situation shall be limited to Nebraska City Public Schools staff who have received
appropriate training in the use of the AED as per Nebraska state statute or Nebraska City Rescue
personnel responding to an emergency in the school.
Nebraska City Public Schools shall develop and implement a program to maintain and inspect the defibrillators according to the manufacturers guidelines.
All designated staff members will be given training in cardio pulmonary resuscitation (CPR), including the use of the AED and be certified in these skills in the appropriate time frames.
The policy and administrative regulations are to be reviewed and updated as needed.
