PUBLICATIONS PERMISSION FORM - STUDENT
Read this form carefully. Check ALL
appropriate statements. Most likely, ALL FOUR STATEMENTS will
apply to your son/daughter. If we do not receive permission from
you, your son or daughter’s photo and/or name will NOT appear in the
2007-2008 Pioneer Yearbook or in any edition of the school
newspaper,The Otoean.
