Medical History Notification
MEDICAL HISTORY NOTIFICATION
Parents are responsible for notifying the school of any medical condition including asthma, severe allergies, diabetes, seizure disorder, or any other medical condition that may require any significant level of intervention while students are at school. Parents are required to complete an emergency action plan and provided the school with the prescribed medications and supplies necessary to address any potentially severe situations. To file an Emergency Action Plan parents should contact the school nurse at 873-3360. Parents should be reminded to continually keep the school nurse and or school officials updated on any change in health status of students during their school aged years.
