Transition:  A Life Long Journey

 

Transition Is:  PLANNING (Failing to Plan IS Planning to Fail)

  • Independent Living:  Residential Options Available, Support Needed to Live Independently
  • Education/Training:  Self-determination, School-To-Work, Transition into Post Secondary Education, Scholarship Opportunities, Vision Statement, Abilities and Interests, Person-centered Mapping
  • Employment:  Job Shadowing, Work Experience in the Community, Career Planning
  • Community:  Community Resources, Partnerships, Training Sites, Building Community Activities/Clubs/Organizations

When Do I Start?

Initial transition planning must begin by age 14 according to Federal Law (IDEA). Transition planning takes TIME. It cannot happen overnight. By starting at age 14 or before, students and families will:

  • Explore interests in each transition area
  • Understand how the disability may impact life
  • Develop necessary skills for adult living before leaving high school
  • Build on accomplishments, knowledge, experiences, and talents
